Harvard University researchers have 3D-printed human heart tissue on a chip with integrated sensors, marking a major breakthrough for personalised medicine.
Tissue-on-a-chip technology may someday lead to replicating a patient's specific genetic disorder in a laboratory, matching the properties of a disease or even an individual patient's cells for testing and treatment purposes.The results of the research by Harvard's John A. Paulson School of Engineering and Applied Sciences (SEAS) and the Wyss Institute for Biologically Inspired Engineering at Harvard were published in the peer-reviewed journal Nature Materials.
"We are pushing the boundaries of three-dimensional printing by developing and integrating multiple functional materials within printed devices," Jennifer Lewis, Hansjorg Wyss Professor of Biologically Inspired Engineering and coauthor of the study, said in a news release. "This study is a powerful demonstration of how our platform can be used to create fully functional, instrumented chips for drug screening and disease modelling."
While Harvard researchers have developed 3D-bio-printed heart tissue on a chip in the past, the embedded sensors on the new chip allow them to easily record data from multiple tissues at one time and over extended periods, according to Johan Ulrik Lind, first author of the paper and a postdoctoral fellow at the SEAS.
"The data we get from the sensors is the most critical information pertaining to cardiac tissue: the strength of the 'heart beat' and the beat rate," Lind stated in an email to Computerworld. "This enables us to study how the tissue responds to drugs and toxic compounds. Because pharmaceutical research can involve [tests of] thousands of compounds, this is an important improvement over previous work."
Typically, disease research is conducted through clinical studies, which can take years to complete, and testing a single drug compound can cost more than $2 billion, according to the Wyss Institute.
In addition, thousands of animals are typically killed in order to test new drug treatments - a process that often fails to truly replicate a human response to the therapy because animals do not accurately mimic human pathophysiology.
The Harvard researchers first fabricated microchips that recapitulate the micro-architecture and functions of living human organs, which so far have included lungs, intestines, kidneys, skins and bone marrow.
The microchips, called "organs-on-chips," are made of a clear flexible polymer about the size of a computer memory stick that contain hollow micro-fluid channels connected with cell-lined artificial blood vessels. Researchers are then able to apply mechanical forces to the chip to mimic the physical micro-environment of living organs, including breathing motions in lungs and peristalsis-like deformations in the intestine.
The researchers can literally take skin cells from a patient that suffers from a heart complication and turn the cells into stem cells, which can then be made into cardiac cells, Lind explained. Those heart cells then match the exact genetic profile of the patient.
"By making a heart-on-a-chip device with those cells, we could then investigate potential therapies for that specific patient," Lind said. "This might sound a bit like science fiction, but our lab has already been part of investigating patient specific therapies using heart-on-a-chip devices in a previous study."
The latest 3D, bio-printed heart-on-a-chip can be quickly fabricated using an automated process, but it also allows for customisation of individual patients by using their own cells, which enables researchers to collect reliable data for short-term and long-term studies.
"Because the entire device is 3D printed, we can easily change design to accommodate, for instance, a specific cell source. This will in the future enable us to tailor the device to heart tissue derived from induced stem cells from a specific patient," Lind said. "This new programmable approach to building organs-on-chips not only allows us to easily change and customise the design of the system by integrating sensing but also drastically simplifies data acquisition."
The Wyss Institute is not alone in its development of 3D bio-printing for drug testing.
Along with research at other universities, 3D bio-printing technology for medical research and therapeutic applications has also been accomplished by San Diego-based Organovo, which printed the first human liver on a chip in 2014.
Organovo also began printing human skin, which could be made from a patient's own cells and used for grafting.

Researchers at Brigham and Women's Hospital (BWH) and Carnegie Mellon University have introduced a unique micro-robotic technique to assemble the components of complex materials, the foundation of tissue engineering and 3D printing, described in the Jan. 28, 2014, issue of Nature Communications ("Untethered micro-robotic coding of three-dimensional material composition").
Tissue engineering and 3D printing have become vitally important to the future of medicine for many reasons. The shortage of available organs for transplantation, for example, leaves many patients on lengthy waiting lists for life-saving treatment. Being able to engineer organs using a patient's own cells can not only alleviate this shortage, but also address issues related to rejection of donated organs. Developing therapies and testing drugs using current preclinical models have limitations in reliability and predictability. Tissue engineering provides a more practical means for researchers to study cell behavior, such as cancer cell resistance to therapy, and test new drugs or combinations of drugs to treat many diseases.
The presented approach uses untethered magnetic micro-robotic coding for precise construction of individual cell-encapsulating hydrogels (such as cell blocks). The micro-robot, which is remotely controlled by magnetic fields, can move one hydrogel at a time to build structures. This is critical in tissue engineering, as human tissue architecture is complex, with different types of cells at various levels and locations. When building these structures, the location of the cells is significant in that it will impact how the structure will ultimately function. "Compared with earlier techniques, this technology enables true control over bottom-up tissue engineering," explains Tasoglu.
Tasoglu and Demirci also demonstrated that micro-robotic construction of cell-encapsulating hydrogels can be performed without affecting cell vitality and proliferation. Further benefits may be realized by using numerous micro-robots together in bioprinting, the creation of a design that can be utilized by a bioprinter to generate tissue and other complex materials in the laboratory environment."
Our work will revolutionize three-dimensional precise assembly of complex and heterogeneous tissue engineering building blocks and serve to improve complexity and understanding of tissue engineering systems," said Metin Sitti, professor of Mechanical Engineering and the Robotics Institute and head of CMU's NanoRobotics Lab.

Experts assume that healthcare only accounted for 1.6 percent of all investments made into the $700 million 3D printing industry. However, that number is expected to grow to 21 percent over the next 10 years.
The latest research shows an even more drastic development for health and medicine. Using 3D printing for medical applications could amount to a market value of $2.13 billion by 2020, says market research company MarketsandMarkets.com. Applications such as dental implants have already been very successful commercially: It’s assumed that around 50,000 custom-fit Invisalign braces are printed on a daily basis.

Two highly motivated stakeholders (the patient and her husband) had access to the patient’s medical data and were able to enhance its value using new era tools (software and 3D printing) and bring it to the docs – clearer – so they could better apply their clinical skills. Specifically, the physicians who read the scans before had not seen the situation clearly. With the tumor printed, a better picture emerged. That is adding value in medicine.

Enormous technological changes in medicine and healthcare are heading our way. These trends have a variety of stakeholders: patients, medical professionals, researchers, medical students, and consumers. They are important because of the impact they will likely have on all of us at one time or another. To get an overview of the trends in healthcare technology, we turned to Dr. Bertalan Meskó, medical futurist and author of The Guide to the Future of Medicine: Technology and the Human Touch. In it, he identifies several areas that he believes will shape the future of medicine and healthcare for decades to come.
Meskó’s predictions easily fall into two categories. The first group of trends, descried below, involve concepts already underway today, or those that will likely have an impact on us in the near future. (Part 2 of this article discusses a second group of tech trends that are still several years away or in much earlier stages of development).
Gamifying health Games are ubiquitous on our computers and phones, and increasing numbers of them are designed to have a positive impact beyond simply killing time. Combining fun and games into healthcare apps can motivate the patient and collect data needed to make informed decisions on daily activities that contribute to one’s health. “An estimated 50% of patients with chronic diseases do not follow the prescribed treatment,” says Meskó. “Gamified health tracking creates an environment that keeps the patient from straying from the appropriate therapy path.”
Empowered patients Patients will become equal partners with their caregivers. Healthcare is moving beyond the hospital, and shifting towards patient self-knowledge and empowerment. The Internet has led to many people (for better or worse) researching their symptoms and diagnosing and treating themselves. While that extreme should be avoided whenever traditional healthcare providers are available, there’s no way to put the genie back in the bottle in terms of patients educating themselves. Rather, healthcare professionals should embrace the change and guide patients in participating in their own care. New technologies will finally help medical professionals focus more on the patient as a human being instead of spending time hunting down pertinent information. They will be able to do what they do best – provide care with expertise. In turn, patients will get the chance to be equal partners in their healthcare. As Meskó puts it, “Healthcare cannot really advance without physicians letting their patients help themselves.”
Telemedicine and remote care Home healthcare services and innovative technology will allow for doctor-patient connectivity where it had not been previously possible, saving both lives and money. Patient monitoring before, during, and after a procedure can now include autonomous robots, such as iRobot’s RP-VITA.
Re–thinking the medical curriculum Medical schools will prepare future physicians for a world full of e-patients and dazzling technology. It takes many years to go from studying to practicing medicine. During that time, what students are learning is constantly changing in the real world. The old-fashioned textbook is a static learning piece in a dynamic professional field with integrated, innovative technology. Digital classrooms will create new connections between students and healthcare professionals and allow for access to the most current information and resources.
Surgical and humanoid robots Robotic-assisted surgery enhances the skill of the surgeon and allows for less invasive procedures. Advanced robots will be able to perform an operation from continents away, with precision beyond what a surgeon’s hand can do. Robots may never fully take over a surgical room due to their weak versatility and adaptability compared to humans, but they will become much more integrated into surgical teams.
Genomics and truly personalized medicine DNA analysis will become a standard step when prescribing medicine or treatment, to ensure it is personalized and optimized for that particular patient’s metabolic background. This kind of specificity, according to Meskó, “will make it possible to define disease in terms similar to GPS coordinates.”
Body sensors Technology is allowing us to measure critical health parameters in convenient and inexpensive ways. Tiny, wearable, sensors collect data without inferring with our daily lives in order to make better, more informed quantifiable decisions. Electronic clothing paired with sensors is one outlet used to collect such data.
Medical tricorders and portable diagnostics The fictional medical tricorder from Star Trek is soon to be a reality. Diagnostic procedures are shifting towards devices that are portable and able to be performed from home. Medical mobile applications will be prescribed with patient customization. “The smartphone will be the hub of the future of medicine,” says Meskó, “serving as a health-medical dashboard.”
Do-It-Yourself (DIY) biotechnology Cheaper technology and a DIY spirit are generating a new generation of scientists and engineers who see no limitations in research. Community biology labs are popping up around the world, connecting inventors, amateurs, and anyone curious to experiment with equipment and education. The resulting innovation in biotech has the potential for disruptive solutions that will further change the way medicine is practiced.
